Anaesthetized under ISO and oxygen via mask for PT (3-4% maint). used hot pack on elbow for several minutes before starting PT, no severe swelling. Wrist was very stiff and attempting to work out wrist or extend elbow at all was painful - increased ISO to 4%. Helped a little but still very reactive with elbow extension. Admin half dose of torb and it seemed to help a lot; held elbow stable and was able to achieve full extension in both joints after about a minute of stretches & holds. Could feel some muscle reaction in upper wing but no whole body reactivity. Held at extension a few times before replacing fig 8 wrap.
R foot suture site looks very clean and quiet, sutures intact, no blood on foot or bandage. Left bandage off.

Extremely stiff and reactive, took many reps, 4% ISO and a half dose of torb to get to eventual full extension. Went slow and held elbow at all times. No movement or shifting felt in joint. Elbow more reactive, wrist stiffer. Comfortable full ext @ end

BARP. 17 g LO. Recovery uneventful, a bit out of it when brought back outside.

VET NOTES
Brought in for PT and bandage change.
Shot a quick radiograph of the elbow - the radius is nicely in place. There is inflammation over the ventral elbow.
Put a hot pack on the elbow while looking at the foot. Removed the foot bandage - sutures on the foot are in place and it looks quiet, uninflamed. Cleaned lightly and then just bandaged with corn pad and tape.
I am all right with the bandage staying off of the foot as long as the sutures look all right, but if leaving it on replace in 4 days.
Did gentle PT after a few minutes of hot pack. Carpus and elbow were a little stiff, but relaxed after a minute of so of gentle stretching and holding, protecting and supporting the elbow at all times.
Recheck in 1 week. Continue PT every 2-3 days. Dr Gardner

PT under 3.5% ISO. The elbow is really thick with scar tissue and took several tries to get the elbow to full extention and have the bird not struggle. The wrist went well and didn't have any reaction. Replaced wrap with cast padding, vet wrap and tape.

BARP, footy. NOLO. Condo very dirty. Removed band, slight bruising on L TMT. Don't think this is a reaction to the band, seems more like the band was not closed properly.

VET NOTES
Wrap in place, bird is overall doing well.
Anesthetized for radiographs. One radiograph done with bandage in place - appears that the radius is in place but needed to remove wrap. Removed wrap. The elbow is bruised and puffy, the wing is tight. Repeated radiographs to confirm that elbow is still in place.
Did very gentle PT - stretching and gentle massaging patagial tendon, not really stressing elbow. Focused more on carpus and holding the wing open.
From the beginning of PT to the end the swelling of the left wing improving vastly.
The dorsal right foot between digits 3 and 4 was still firmly swollen. Given the scab on the ventral aspect, concerned that there is some infectious debris trapped in the foot.
Plantar aspect of left foot is pink, concerned that she is putting more weight on left foot.
Moved upstairs for debridement of foot - see surgery notes.
During surgery applied a hot pack to the left elbow, then re-wrapped in padded figure-8.

P: for now - PT every 2-3 days - gentle, under anesthesia. Recheck foot in 4 days (but check bandage daily). Dr Gardner

Positioned in sternal recumbency, gently taped digit 1 against
the TMT to keep it out of the way. Sterile prep of ventral foot,
then draped out everything except the area of concern.
Using forceps and sharp scissors excised scab on bottom of foot.
Slightly widened defect, then explored with forceps and a hemostat.
Pulled out several chunks of infectious exudate that had made
a pocket between the toes at the dorsum of the foot. Flushed
copiously. Explored again until I was sure all gunk was out then
debrided with q-tip and flushed again. Freshened edges of scab
and closed with 3-0 PDS with two simple interrupted sutures.

Examination of dorsal foot showed improvement of puffiness. 
Placed a corn pad over sutures and a ball bandage.

NOLO BAR

Veterinary Exam, Right Foot:

Patient placed under brief general Isoflurane anesthesia for examination of right foot.

Right foot has dorsal soft tissue swelling, mild present at base of digit #4 and mild swelling plantar aspect of digit #4 where it meets with center pad. In the very center of the plantar aspect of the foot there is tiny, 1-2 mm older scab present that is well healed and is beginning to lift up slightly. The central foot pad has minimal soft tissue swelling, if any. Talons and other digits appear normal.

The right foot area was cleaned with chlorhexidine and alcohol three times.
Two fine needle aspirates were collected with 3 cc syringe with 22 gauge needle.
In house cytology performed to evaluate cell sample collected. Minimal fluid present at time of aspirate.

Assessment: Mild soft tissue swelling at base of digit #4 dorsally. Focal plantar lesion as well that is almost fully healed.

Rule out inflammation vs. infection vs previous trauma.

Cytology Results: FNA sample was good sample. Consistent with coccoid bacterial infection of the base of the digit/foot. Red blood cells and random clusters of heterophil white blood cells present with intracellular coccoid bacteria. Negative for fungal or other organisms.

Plan:
Start enrofloxacin orally for 14 day course in food. Continue the meloxicam and tramadol for pain relief related to the left elbow luxation.

Young RTHA found in finder's yard with injured L wing. Finder was able to corral into her empty chicken pen before RRT volunteer arrive to capture. On admission, bird presented BAR and visual. NSF on PE until reaching L wing. Felt swelling at the elbow and took dirty rads with assumption that elbow was dislocated. Anesthetized under 3% iso, rads confirmed radius was dislocated from joint. Was able to manipulate wing and secure radius back into place with some effort. Applied snug padded figure 8 and recaptured rads. Leave wrap on for at least 5 days before starting PT. Wrap will need tape tomorrow. Set up in KR2 kennel. Gave 1 mouse.
